Ichigo only goes hollow when in bankai. (except when he's training in the manga)

Acctually...
He goes Hollow mode one more time that I can think of in the Anime around episode 80 something, but thats the only time, and he was normal Shikai mode.
I've beaten KHII and seen all available Bleach Episodes so nothing is a suprise to me.
