First off, Haruka is Keitaro's aunt. Haruka Urashima is a smoker and is almost always smoking. Haruka is also a drinker, and I'm talking about alcoholic drinks here. Haruka's favorite color is black.
Haruka's favorite sport is gun shooting. Also, Haruka is owner of the Tea House. Haruka has a past with Seta... well, so it is believed. Haruka hates it when Keitaro calls her Aunt Haruka because "aunt" also can mean "old woman".

she is Keitaro's cousin buy birth be he calls her aunt because of the stuff that played out.

Haruka was raised by her grandma Hina because something happened to her parents making her go from Keitaro's cousin to his aunt because being raised by Hina would be like its her child and Keitaro's aunt.

It all depends on the translation as to her direct relation to Keitaro. Since in Japanese, Obasan(aunt) resembles Obaasan(old lady) making the comedy (Haruka smacking him or threatening to smack him) from this word play have no meaning outside of the language.
