For me, the saddest ever is Knight Hunters Eternity but the following also have some very sad endings or major sad parts before you you get to the ending.

I Cried so hard in Elfen Lied. I can't think of any anime off the top of my head that the ending was sad, i mean i've cried before, but only for my own reasons. I mean, i cried at GunSlinger Girl..... I am ashamed.... And i cried at fruits basket.

Yes it was. Out of the entire cast of heroes, three survived, and one of them went insane.

Of course, there are other anime with higher body counts.

Dunbine only had one character survive the final battle. In Ideon, no one was spared, as the entire cast ends up dead, all of them by graphic violent means (save for a baby, whose death is not shown on camera. Oh yes, the entire human race is also wiped out in the battle, along with the entire Buff Clan race. So yeah, nobody wins. Not surprisingly, Zeta Gundam, Aura Battler Dunbine and Space Runaway Ideon all have the same director.

I agree that the ending of these two anime were sad. To add, the end of Paradise Kiss was also sad but it was Grave of the Fireflies that made me cry. The few last eps of Midori no Hibi, too. huhuhu...oh i love those anime.
