Personally, I thought Dream Theater's cover of 'Stargazer' was very average.

Staind's cover of Metallica's "Nothing Else Matters" from MTV Spotlight: Metallica was amazing.
However, any band who covers Queen can be shot

White stripes does a load of amazing covers.

I really like "For What It's Worth" when Rush covered it, I also enjoyed Rush's "The Seeker" but not as much as the original.

The worst cover ever definitely has to be Suicide Silence's cover of "Them Bones" by Alice in Chains. That is honestly some of the worst [censored] I have ever heard.

I laughed my ass off when I heard Children of Bodom cover Oops, I Did it Again.... I liked the Deathstars cover of White Wedding, though I beleive The Murderdolls did a decent cover of that song as well.
