'Happy Birthday' = 'Omedeto'
('Omedeto' also translates to 'Congratulations')

Happy birthday is actually "tanjoubi omedetou".
Tanjoubi = birthday (or its polite form is o-tanjoubi)
Omedetou = congratulations

So for your message, you could write something like:

"(O)tanjoubi Omedetou!
Anata no suenagai shiawase o oinori moushiagemasu, Ayano.
Aijou o komete,
[your name]"

(お)誕生日おめでとう！
あなたの末永い幸せをお祈り申し上げます、あやの。
愛情を込めて、
[your name]
